Respectfully returned to the Commissioner of Pensions Samuel Rankin Richardson Private of Company D, 128th Regiment Illinois Volunteers was enrolled on the 26th day of September 1862, at Camp Butler, Illinois for 3 years and is reported transferred to Company H 9th Illinois and is reported on roll for September and October 1863 present.  Return for October 1863 reports him gained by transfer October of 1863 at Pocahonta, Tennessee.  November and December 1863 present so reported to June 30, 1864.  Transferred to Company C. same regiment and is reported on roll for July and August 1864 absent sick.  September and October present same to June 30, 1865.  Mustered out with Company July 24, 1865.  Return for July 1864 does not report him absent sick or otherwise.

The 128th Illinois Volunteers having during its short period of service of less than five months, became reduced from an aggregate of 860  to 161 men principally by desertion, the remainder were formed into a detachment on or about April 7th 1863, and subsequently transferred to other Regiments for Special Order No. 1 War Department dated Cairo, Illinois April 15th 1863.

No evidence of alleged disability - Mornis report Co. H 9th Illinois Volunteers Volumes October 5, 1864 shows him "returned from hospital."  Books of 128th Illinois Volunteers and Regimental Hospital records of 9th Illinois Volunteers not on file.
